Before participating, read the Guidelines – PDF
Submit the following:

Completed application/expense form - PDF

Original detailed itemized receipts for travel and accommodations

Confirmation from the Undergraduate Medical Education office providing:

Confirmation that you are in good standing;

Rotation location and dates upon completion of your rotation;

Academic year for the eligible rotation;

Social Insurance Number form - PDF

Confirmation of Banking Information - PDF
Please review the expense guidelines before booking travel and accommodations to ensure all guidelines are followed.
Submit appropriate supporting documents and retain a copy of all the documentation for your records.
